Information for Professionals

We provide a simplified central referral point for professionals who want to make a referral to one of our services. It is designed to be a continued resource to navigate options and support available throughout LLR. You can call our dedicated business line or email us, using the secure or non-secure email addresses, dependent on the nature of your enquiry.

From 1 July 2019, you can refer adults across Leicester, Leicestershire and Rutland using our NEW online form below. For other services use the downloadable forms  or telephone the Business line.

  • Referrals can be made online. referrals.uava.org.uk
  • Referrals can be phoned through to the Business Line
  • Referrals can be sent via email
  • Referrals can be faxed/posted

We are currently receiving an unprecedented number of calls at the moment. If your call is not answered, kindly leave your name and number on the answerphone and we will call you back as soon as we are able.  This line is for business calls from professionals only. Calls to this number are screened.

How does the referral process work?

Any professional can make a referral for an individual or family using the above options. We will then assess the needs of the person or persons and any risks, in order to provide tailored support.

You can find the referral form and the DASH risk assessment form available to download below.

Focusing on improving safety, health and well-being and reducing risk, we will work closely with partner agencies to ensure a wider co-ordinated community response.

Services are available across LLR (Leicester, Leicestershire & Rutland) with additional services available for Leicester City residents only.
